The admission process

Admission to a sub-acute or nursing facility can be a stressful and time-consuming process. It comes at a time when families are often taking part in a major change that has occurred in the life of their loved ones. At Oakwood, a dedicated admissions team works closely with families, to process all aspects of admission including helping secure whatever benefits a resident may be eligible to receive from Medicare, Medical Assistance, the grant, Medicaid or other insurances. The Admissions team will assure that the entire process is a pleasant experience, thoroughly explain care related areas and alleviate the stress often incurred when admitting a loved one to a healthcare facility.

A great deal of background work and preparation is done by our professional staff prior to and upon admission to assure that a resident is given the best care from the very first moment of arrival. This includes a complete evaluation of the resident’s medical history, an assessment of therapeutic needs, as well as an analysis of lifestyle such as nutrition, exercise, and social needs.

Once admitted to Oakwood, the comprehensive team of clinicians will assess, evaluate and collaborate, to design a carefully crafted individualized plan of care with specific objectives and goals outlined, to insure that every aspect of a resident’s needs are addressed and met. Throughout, Social Service will maintain contact with the resident and family to continuously work toward Oakwood’s goal of providing excellent care and patient satisfaction, throughout your journey along the road to recovery.

To arrange for admission to Oakwood

From the Hospital – Inform the hospital social worker or case manager that you have chosen Oakwood for your post-hospital care. They will send medical information and communicate with the Oakwood Admissions staff to coordinate a transfer. Also, call the Oakwood Admissions office at 215.673.7000 to notify them of the pending admission. The admissions team will collaborate with you, the hospital and your attending physician to coordinate transfer to Oakwood.

From Home – just call the Oakwood Admissions office at 215.673.7000 – they will guide you through the process and coordinate transfer to Oakwood.

From an Acute Rehab, L-TAC or other Nursing Facility – notify the social worker or case manager that you wish to transfer to Oakwood. They will contact the Oakwood Admissions office who will work closely with the other facility to facilitate transfer to Oakwood. Also, call the Oakwood Admissions office at 215.673.7000 to notify them of your desire to transfer to Oakwood. The admissions team will collaborate with you and the other facility to coordinate transfer to Oakwood.
